Â鶹AV Care is a health care access program through Â鶹AV Health + Hospitals. It provides comprehensive health care services in all five boroughs to New Yorkers who are not eligible for or who cannot afford insurance. 

Membership includes:

• Access to Â鶹AV Health + Hospitals healthcare services such as primary care, specialty care, and behavioral health
• A dedicated primary care provider
• Access to affordable medications day or night
• 24-hour customer service line (646-Â鶹AV-CARE)
• A unique Â鶹AV Care membership card with clear information about the specific no or low-cost fees the member will be expected to pay when they get health care

Â鶹AV Care is available to New Yorkers living in the five boroughs for six months or more who want to receive care at Â鶹AV Health + Hospitals facilities.
